All intake paperwork must be completed through our Electronic Health Record system, the Practice Better Client Portal. If you are being referred by another provider, please ensure you provide a Referral Form that includes the referring diagnosis.
We accept payment via cash, check, or credit card. All payments are made from invoices through Square via our EMR system, Practice Better. Please note that a fee, equal to the Returned Check Fee, will be assessed for any checks returned for non-payment.
Yes, you can request a Superbill at the time of service. We will provide one shortly after the session, allowing you to submit it to your insurance for potential reimbursement.
